1. Introduction
Welcome to Our June LLC dba June (“Company,” “we,” “us,” or “our”). These Terms and Conditions (“Terms”) govern your use of our at-home Botox services (“Service”). By booking an appointment, you agree to comply with and be bound by these Terms.
2. Eligibility
To use our Service, you must:
Be at least 18 years old.Provide valid identification upon request.Disclose any medical conditions, allergies, or contraindications before treatment.
3. Services Provided
Our June LLC provides at-home Botox injections performed by licensed medical professionals.Services are limited to Botox treatments as outlined in the consultation.The results of Botox vary, and we do not guarantee specific outcomes.
4. Appointment Booking and Payment
Appointments must be scheduled in advance through our website or customer service.Full payment is required at the time of booking unless otherwise specified.We accept credit/debit cards and other approved payment methods.Any additional fees, including travel fees, will be disclosed prior to booking.
5. Cancellation and Refund Policy
Cancellations must be made at least 24 hours before the scheduled appointment to receive a full refund.Late cancellations (less than 24 hours before) may be subject to a cancellation fee of $50. No-shows will be charged the full amount of the service.Refunds are not provided for completed treatments.
6. Client Responsibilities
You must provide a clean and safe environment for the procedure.You must follow all pre-treatment and post-treatment instructions provided by our professionals.You agree to disclose any medications or health conditions that may impact the treatment.
7. Health and Safety
Our providers follow strict hygiene and safety protocols.If a provider determines that treatment is unsafe due to medical history or other concerns, they reserve the right to refuse service.We do not provide Botox treatments to pregnant or breastfeeding individuals.
8. Liability Waiver
You acknowledge that all medical procedures carry risks, including but not limited to bruising, swelling, or adverse reactions. Our June LLC and its providers shall not be held liable for any complications, side effects, or dissatisfaction with the results.You agree to seek immediate medical attention if you experience an adverse reaction and notify us promptly.
9. Privacy Policy
Your personal and medical information will be kept confidential in accordance with HIPAA and applicable privacy laws.We do not share your information with third parties except as required by law or with your explicit consent.
10. Governing Law and Dispute Resolution
These Terms shall be governed by the laws of Delaware.Any disputes arising from these Terms shall be resolved through arbitration in Delaware, unless otherwise required by law.
11. Amendments
Our June LLC reserves the right to modify these Terms at any time.Continued use of our Service after modifications indicates your acceptance of the updated Terms.
